Choose how to manage your care

If you’ve been assessed as eligible for care and support from the council, we will then agree on a plan for your care with you.

This plan includes how your support will be provided and who will provide it. We will discuss all the options available to you. We aim to complete your plan within 28 days of your assessment.

You can ask us to arrange your care for you or you can use a direct payment so that you can arrange your care and support yourself.
